How to Order: Mail Service Pharmacy

Before ordering your first 90-day supply
It is always recommended
to try physicians' samples, or a 30-day supply, before requesting a 90-day supply due to side effects that could occur. Some employers require an initial 30-day period. Please verify with your employer, or contact the PBM Plus member help desk, to determine how your particular plan handles mail
service prescriptions.
If you need to fill a prescription immediately
Ask your doctor to write two prescriptions: one for a 14-day supply and the second prescription for a 90-day supply. Take the 14-day
supply to your local pharmacy to be filled, and mail the 90-day prescription to PBM Plus Mail Service Pharmacy.
How to order refills
Notify PBM Plus through one of the following options to receive an authorized refill of your medication currently on record.

- By Phone – Call (800) 736-0403 and talk with a customer service representative. Be
sure to give your customer service representative your PBM Plus member ID number and prescription number you wish to refill.
- By Mail – If you wish to reorder by mail, complete a Printable Order Form (pdf) and mail it to: PBM Plus Mail Service Pharmacy, 300 TechneCenter Drive, Suite C, Milford, OH 45150.
Reorder forms are enclosed with initial and subsequent orders. Please allow 14 business days from the time you mail your order for delivery.
- By Fax – If you are paying by credit card, you may fax the order form to fax (888) 868-2854.
Please be aware that new prescriptions can only be faxed to the Mail Service Pharmacy from the physician's office.
